Janus kinase (JAK) is a family of intracellular, nonreceptor tyrosine kinases that play an important role in cytokine- mediated signaling. This family includes four kinases: JAK1, JAK2, JAK3 and TYK2 (Tyrosine Kinase 2). Various studies have suggested that regulation of JAK activity is a promising approach to combat numerous diseases such as immune deficiency and cancer. Upon binding of a cytokine to its corresponding receptor, the receptor changes conformation and recruits two JAK molecules, which phosphorylate the receptor and activate a signal that results in STAT dimerization and translocation to the nucleus. This results in expression of pro-inflammatory cytokines and growth factors. For example, IL-12 and IL-23 receptor pathways require a TYK2/JAK2 heterodimer. TYK2 is also required for type interferon and IL-10 signaling. TYK2 has been closely linked to psoriasis, with individuals that have loss-of-function mutations being protected from this disorder. This points to the possibility of inhibiting TYK2 as a treatment option. Several TYK2 inhibitors have been developed for the treatment of psoriasis, hidradenitis suppurative, IBD (inflammatory bowel disease), dermatomyositis and SLE (systemic lupus erythematosus). Deucravacitinib, an allosteric inhibitor that binds to the pseudokinase domain, has been approved in 2022 for the treatment of plaque psoriasis. This approval indicates the potential of targeting TYK2 to treat inflammatory disorders.
